In this Distinguished IEEE SPS lectu re\, we will see how this transition is becoming reality. Advances in AI a nd large models\, multimodal sensing\, speech and language processing\, af fective and behavioral computing\, and wearable intelligence are enabling AI systems that can monitor health unobtrusively\, detect subtle changes e arly\, and deliver personalized recommendations—right where people live their daily lives. By shifting the focus from treatment to prevention\, AI has the potential to reduce disease burden\, improve quality of life\, an d extend healthy years of living. Yet\, realizing this promise requires mo re than technological breakthroughs. We will discuss how to ensure transpa rency\, privacy\, and fairness when AI moves into the intimacy of the home \, and how to align innovation with the rigor and trustworthiness expected in world-class clinical settings such as the Mayo Clinic. We will also ex plore how the strengths of clinical excellence and AI innovation can build a health system where care is proactive\, patient-centered\, and seamless ly integrated into everyday life.
