Artificial Intelligence (AI) in health is entering a new era. While much attention has been given to its role in clinics—enhancing diagnostics, imaging, and decision support—the real home run lies beyond hospital walls: bringing AI into people’s homes to support prevention, early detection, and continuous care. In this Distinguished IEEE SPS lecture, we will see how this transition is becoming reality. Advances in AI and large models, multimodal sensing, speech and language processing, affective and behavioral computing, and wearable intelligence are enabling AI systems that can monitor health unobtrusively, detect subtle changes early, and deliver personalized recommendations—right where people live their daily lives. By shifting the focus from treatment to prevention, AI has the potential to reduce disease burden, improve quality of life, and extend healthy years of living. Yet, realizing this promise requires more than technological breakthroughs. We will discuss how to ensure transparency, privacy, and fairness when AI moves into the intimacy of the home, and how to align innovation with the rigor and trustworthiness expected in world-class clinical settings such as the Mayo Clinic. We will also explore how the strengths of clinical excellence and AI innovation can build a health system where care is proactive, patient-centered, and seamlessly integrated into everyday life. By highlighting both transformative opportunities and pressing challenges, this talk offers a vision for the future of AI in health—where the clinic’s standards meet the home’s realities, and prevention becomes the true frontier of care.
